patient. If a copy of discharge orders and other information is faxed, it must include a cover sheet with a confidentiality statement. 6. Post-acute Services Providers (including BH Providers) are not permitted to solicit any patient or otherwise approach any patient prior to be being chosen by such patient. Should any BH employee observe solicitation of Patients by representatives of Post acute Services Providers, he/she should report it to the facility CEO (or designee) immediately. The facility CEO should remove the representative of the Post-acute Services Provider from the patient’s room and discuss the facility’s “no solicitation” policy with the person. If this practice occurs on a repeat basis, the facility should take stronger measures to protect its patients. 7. Non-BH Providers are permitted to enter patient units and other non-public areas of BH facilities only for the purpose of visiting the patient who has chosen that Non-BH Provider or requested the opportunity to speak with that Non-BH Provider, or to provide in-service presentations as set forth in Section B.2 below, and must leave the patient unit or non-public area immediately upon completion of the patient visit or in-service presentation. Representatives of BH Providers may enter patient units and other non public areas of BH facilities without restriction, but are subject to the prohibition against soliciting or directly approaching patients. 8. Representatives of Non-BH Providers seeking to regularly visit facilities shall preregister utilizing the designated electronic registration system for that facility and print and wear a visitor badge from this system at all times while in the hospital. This requirement pertains to the Arizona Community Division, Academic Division, Northern Colorado Medical Center, Banner Fort Collins Medical Center, McKee Medical Center and Banner Payson Medical Center only. 9. Representatives of Non-BH Providers seeking to regularly visit BH facilities shall review Banner policies and return appropriate attestation documents, provided by Banner, sign in at the facility-designated areas at every visit to obtain a visitor’s badge and must wear it in addition to their company-provided identification indicating the provider with whom they are affiliated. This requirement pertains to Banner Churchill Community Hospital, Banner Lassen Medical Center, East Morgan County Hospital, Ogallala Community Hospital, Page Hospital, Platte County Memorial Hospital, Sterling Regional MedCenter, and Washakie Medical Center only. Representatives of BH Providers must wear their identification badges at all time. 10. At the discretion and upon the approval of Care Coordination, unregistered, Non-BH Providers chosen by the patient may be granted access and a temporary identification badge to facilitate the safe transition of that patient. If a printed badge is unavailable, the visitor shall be asked for appropriate identification, such as a business card or other employer-provided identification. 11. In the event a Non-BH Provider visits a patient care area without the BH-provided visitor’s badge, they shall be asked to leave the area until they have obtained the appropriate visitor’s badge. C. Marketing Access to BH Employees by Non-BH Providers. 1. BH will respect patient choice with respect to selection of a provider of Post-acute Services. Accordingly, patients will be given the opportunity to choose providers of Post acute Services in accordance with the BH Policy “Patient Referral for Home-Based Services”. Respecting patient choice does not, however, obligate BH to permit Non-BH Providers to market or to otherwise solicit business from BH employees or patients while admitted in a BH facility. 2. In areas/communities where a BH Provider is available to provide such services, BH will not provide access or opportunity for representatives of Non-BH Providers to advertise, market, address or otherwise solicit business from BH case managers, social workers,
