Upon confirmation of completeness, the UPRT Request Form and any additional information will be submitted to UPRT for review. 4. In reviewing the proposed Secondary/Additional Professional Activity, UPRT will consider the following factors: a. Whether the requested Secondary/Additional Professional Activity is already a part of the Employed Practitioner's Primary Activities. Whether the requested Secondary/Additional Professional Activity is consistent with Banner's Covered Professional Activity philosophy, as outlined in Article III, Section B.1. Whether UPRT is willing to allow the Employed Practitioner to engage in the requested Secondary/Additional Professional Activity. b. c. 5. Following a final recommendation/approval by UPRT and approval by the AMC, if approval by the AMC is required, the Physician Compensation Program Director, or his/her designee, will notify the appropriate operational contact within B—UMG in writing as to whether the requested Secondary/Additional Professional Activity has been approved or denied, and, if approved, any conditions that may have been placed upon such approval. 6. If the requested Secondary/Additional Professional Activity is approved, the appropriate operational contact will initiate a request for a written amendment to the Employed Practitioner's Employment Agreement in accordance with the then current process identified by the Legal Department, which request will include a copy of the UPRT Request Form and written evidence of UPRT and/or AMC approval. 7. If the approved Secondary/Additional Professional Activity is to be provided to Banner, the University of Arizona, BMG, BMGC or BUCS, and not to B—UMG, the appropriate operational contact also will initiate a request for an Intra-Entity Agreement/Special Provision between B—UMG and Banner, the University of Arizona, BMG, BMGC or BUCS in accordance with the then-current process identified by the Legal Department, pursuant to which B—UMG will provide the services of the Employed Practitioner to Banner, the University of Arizona, BMG, BMGC or BUCS in connection with the approved Secondary/Additional Professional Activity. 8. An Employed Practitioner may not begin to provide any Secondary/Additional Professional Activity to B—UMG, another Medical Group, Banner or the University of Arizona until (a) the proposed Secondary/Additional Professional Activity has been approved by UPRT and/or the AMC, (b) the written amendment to the Employed Practitioner's Employment Agreement has been fully executed by both the Employed Practitioner and by B—UMG, and, (c) if applicable, the related Intra-Entity Agreement/Special Provision has been fully executed by both B—UMG and Banner, the University of Arizona, BMG, BMGC or BUCS, as the case may be. 9. The Medical Group may terminate an Employed Practitioner's obligation to provide any Secondary/Additional Professional Activities at any time upon written notice to such Employed Practitioner. d. Whether the Employed Practitioner will receive Secondary/Additional Compensation for the proposed Secondary/Additional Professional Activity, and, if so, the manner in which the Employed Practitioner will receive the Secondary/Additional Compensation. Whether the total compensation to be received by the Employed Practitioner, including any Secondary/Additional Compensation, is consistent with fair market value. e.
