Title : Secondary/Additional Professional Activities and Outside Activities of Employed Practitioners Policy and Procedure Number : 3977, Version : 1 Professional Activity Request Form and all requested supporting documentation to Banner Research for further consideration. Banner Research will review the proposed Covered Professional Activity and advise SPAC as to whether the proposed Covered Professional Activity should be approved or denied. 8. Following a final decision by SPAC, the Employed Practitioner will be notified as to whether the requested Covered Professional Activity was approved or denied, and, if approved, whether the requested Covered Professional Activity is a Secondary/Additional Professional Activity will be performed within the scope of the Employed Practitioner's employment by the applicable Medical Group or an Outside Activity that will be performed outside the scope of the Employed Practitioner's employment by the applicable Medical Group. The written notice also will specify whether any conditions were placed upon the approval of the requested Covered Professional Activity. a. If the requested Covered Professional Activity is denied, the chair of SPAC, or his/her designee, will prepare and send to the Employed Practitioner a letter notifying the Employed Practitioner that the requested Covered Professional Activity was denied and advising the Employed Practitioner of any ability to request reconsideration pursuant to Article IV, Section H. If the requested Covered Professional Activity is approved as an Outside Activity that will be performed outside the scope of Employed Practitioner's employment by the applicable Medical Group, the chair of SPAC, or his/her designee, will prepare and send to the Employed Practitioner a letter notifying the Employed Practitioner that the requested Covered Professional Activity was approved as an Outside Activity that will be performed outside the scope of the Employed Practitioner's employment by the applicable Medical Group and advising the Employed Practitioner of any conditions were placed upon the approval. If the requested Covered Professional Activity is approved as a Secondary/Additional Activity that will within the scope of the Employed Practitioner's employment by the applicable Medical Group, the chair of SPAC, or his/her designee, will forward the Covered Professional Activity Request Form and written evidence of SPAC's approval to the appropriate operational contact within the Medical Group to initiate a written amendment to the Employed Practitioner's Employment Agreement, if any, in accordance with the then-current process identified by the Legal Department. If required, the appropriate operational contact also will initiate a request for an agreement between the applicable Medical Group and the Outside Entity in accordance with the then-current process identified by the Legal Department, pursuant to which the applicable Medical Group will provide the services of the Employed Practitioner to the Outside Entity in connection with the approved Secondary/Additional Professional Activity. 9. An Employed Practitioner may not begin to perform any Covered Professional Activity to an Outside Entity until the proposed Covered Professional Activity has been approved by SPAC. In addition, if the approved Covered Professional Activity is being provided to an Outside Entity as a Secondary/Additional Professional Activity within the scope of the Employed Practitioner's employment, the Employed Practitioner may not begin to perform the Secondary/Additional Professional Activity until (a) a written amendment to the Employed Practitioner's Employment Agreement has been fully executed by both the Employed b. c. d.
